Canada - Paperwork

Greetings All,

This is the first of several webpages on Canada; as noted in the title, this one concerns needed paperwork. Everyone needs to pay particular attention to this page, but it is most critical for drivers and Scouts who are not accompanied by a parent.

Note: Canadian Fishing Licenses are required, and will be acquired in Canada.

Needed by Everyone

1) Killarney Outfitters Release Form (will be available at the Monday Night Prep Meeting; must be signed by all attendees, plus by parents of attending minors.)

Needed by Drivers

2) Valid U.S. Driver's License, or International Driver's License; the latter is preferred, but is NOT mandatory.

3) An insurance card VALIDATING the insurance on the vehicle you are driving in Canada. This is NOT just your regular insurance form, but rather a special card, good for one year only, which your insurance company provides to you upon request. YOU MUST CALL YOUR INSURANCE AGENT/COMPANY - PREFERABLY TOMORROW - AND REQUEST THIS CARD. It usually takes a few days to type up and get in the mail to you, so calling early tomorrow morning is a very good idea.

Needed by Adults

4) A valid passport or NOTARIZED COPY of your birth certificate (that is, a photocopy isn't adequate). If anyone who is attending is NOT a native-born U.S. citizen, bringing both the passport and the birth certificate are recommended, along with your Naturalization Papers. If you don't have these items (especially the latter), getting back into the United States could become a sudden, serious problem.

5) A copy of the Troop 111 Medical Form for Adults. Attaching a copy of your most recent physical is an excellent idea.

6) Your medical and dental insurance cards. Note well any restrictions on their use in Canada. If there are problems, call your insurance company HERE, THIS WEEK, to clarify or to purchase additional coverage.

7) If you are taking any controlled medications, a copy of your current prescription(s).

Needed by Scouts (ALL)

8) A valid passport, driver's license or birth certificate.

9) The Troop 111 Permission Slip, signed, for this trip (will be available at the Monday Night Prep Meeting.)

10) A copy of the Troop 111 Medical Form for Scouts (we should have all of these, but if you took yours for some reason after summer camp, we need it back!) Attaching a copy of your most recent physical is an excellent idea.

11) Your family's medical and dental insurance cards. Parents - Note well any restrictions on their use in Canada. If there are problems, call your insurance company HERE, THIS WEEK, to clarify or to purchase additional coverage.

12) If a Scout is taking any controlled medications, a copy of the current prescription(s).

Needed by Unaccompanied Scouts

13) Troop 111 Unaccompanied Minor Permission Form (will be available at the Monday Night Prep Meeting); this is a blanket permission form to transport your son in and out of Canada, give him medical treatment as needed in or out of Canada, and otherwise to act in loco parentis for any other issues. It will also have your contact information for the period that we are in Canada. NOTE THAT IS *MUST* CONTAIN A PHOTO OF YOUR SON, AND THAT YOU MUST SIGN ACROSS THE PHOTO AND ONTO THE DOCUMENT; therefore, you will need to bring a recent photo to the Monday Night Prep Meeting. Note that a typical Scout photo (closeup, cropped as needed) is perfectly adequate - no need to go buy a passport photo.
