The Basics of the Permission Slips

Every permission slip has 4 parts--and sometimes there is an extra line for specific information (like on the Projectoree and white water rafting slips). It saves us a lot of guess work to have all parts completed (legibly):

Part 1--please put in the scout's FULL name. We have multiple Mikes, Brians, Nicks, etc., and the parent signature is often "stylistic" (i.e., illegible).

Part 2--please give an emergency contact number just in case.

Part 3--VERY IMPORTANT--tell us what you will do. If you put your name down for "will attend" it means you are going on the trip and camping with the scouts--the whole 9 yards--and you will be charged (just as every person, including Dr. Bob, is charged for a trip). If all you plan to do is drive, put your name down for "will drive," not "will attend". We often can cover driving from adults who are attending, but sometimes (Projectoree is one) we need extra drivers. The troop doesn't charge those who drive only. And if you can't do anything, please check the "cannot attend or drive" line so we know for sure. You will be reimbursed for gas. Fill your tank before you come, then fill up again immediately after and TURN THE RECEIPT IN!

Part 4--payment--give us a check (made out to Troop 111) for the estimated amount times the number of attendees (see #3) or mark troop credit; if we don't get a check, we charge your troop account.

If there is another line for special info, please fill that in too. Finally, write any special info in the margins--have to join a trip late? come home early? have a special timing problem? anything like that, just jot us a note--if there is no special info, we assume you are available at the times listed for the trip.
